Note - all forms must be completely filled out with complete address, phone and contact information. Forms missing information
will not be processed. STEP
1: All club members are required to sign up for a membership in USA Track and Field:
The USATF Club Number for Northstar United is: 319 If you plan on participating
in the AAU meet in Iowa or Wisconsin you must also sign up for an AAU membership:
The AAU Club Number for Northstar United is: MNAT5FAKA8 STEP 2: Fill out the club registration
form - make sure you put your USATF or AAU (if participating in the Iowa or Wisconsin meet) membership number on this form.
Forms missing your USATF number will not be processed until we have a valid membership number. Please make sure you include
a copy of your child's birth certificate. Membership cost is $130 for one family member and
$115 for each additional family member.

STEP 3: Fill out the uniform order form
- the club will issue the top with the required $25 deposit. If your child has shorts from last season that still fit they
can wear them again this season. If they need new shorts fill out the shorts section and include $18 for the uniform shorts.
In addition all forms must be signed with an agreement that the parents and athletes agree to abide by the club's uniform
policy.
